When Artificial Tears Aren't Enough

Most people try the basics first: over-the-counter artificial tears, warm compresses, maybe a humidifier at home. For mild, occasional dryness, that's often enough. But for many patients, dry eye is a chronic, progressive condition — frequently driven by meibomian gland dysfunction, ocular rosacea, Demodex blepharitis, or inflammation on the eye's surface — and conservative measures alone simply can't reach the actual source of the problem.

If you've been using drops for months or years without lasting relief, that's not a sign to try yet another over-the-counter brand — it's a sign your dry eye needs a real diagnostic work-up and a treatment plan built around what's actually causing it.

We built a dedicated, state-of-the-art Dry Eye Center at our North Miami location specifically for patients whose dry eye hasn't responded to conservative treatment elsewhere. It's equipped with advanced diagnostic technology to identify exactly what type of dry eye you have, and the in-office treatments to address it directly — all in one focused setting.

Thermal Treatments: TearCare & ThermalOne Touch

The majority of chronic dry eye cases involve meibomian gland dysfunction — the tiny oil-producing glands along your eyelids become blocked, so the oil layer of your tear film breaks down and your tears evaporate too quickly. We treat this directly with in-office thermal therapies:

  • TearCare delivers controlled, targeted heat to the eyelids to soften and liquefy hardened gland secretions, followed by manual expression to clear the blockage and restore healthy oil flow.
  • ThermalOne Touch combines gentle heat with light pulsation to unclog blocked meibomian glands, improving the quality and stability of your natural tear film with a quick, comfortable in-office visit.

Both treatments are performed right in our office, typically take well under an hour, and are frequently paired with IPL for a complete treatment plan.

The Rest of Your Toolkit

IPL and thermal treatments address the most common root causes of dry eye, but a complete treatment plan often draws from several tools at once. Depending on what your evaluation shows, we may also recommend:

BlephEx

An in-office lid-margin exfoliation treatment that removes the biofilm, bacterial debris, and excess bacteria that build up along the eyelid margin and lashes — the primary treatment for Demodex blepharitis, a common and often overlooked contributor to chronic dry eye and lid irritation.

Punctal Plugs

Tiny, virtually unnoticeable plugs placed in the tear drainage ducts (puncta) to slow how quickly your natural tears — and any drops you use — drain away, keeping the eye's surface better lubricated for longer. A simple, quick, in-office addition for patients who need extra moisture retention.

Prescription Dry Eye Drops

For dry eye driven by inflammation, artificial tears alone often aren't enough. We prescribe and manage advanced anti-inflammatory dry eye medications tailored to your specific presentation, working alongside in-office treatments rather than in place of them.

Amniotic Membrane

For severe or non-healing cases, a therapeutic amniotic membrane can be placed directly on the eye's surface to calm inflammation and accelerate corneal healing — jump-starting recovery when other treatments haven't been enough.

Serum Tears

For the most stubborn, treatment-resistant cases, we offer autologous serum tears — drops created from your own blood serum, rich in the same natural growth factors and nutrients found in real tears, custom-formulated for eyes that haven't responded to conventional therapy.
